To obtain credit approval for all new purchase orders with Cevi Med, it's essential to complete this credit application in its entirety.
Required documents include:
- The approval process may take up to two weeksdepending on your credit history.
- Should you need to make any alterations to your application after submission, a new application form must be filled out, and the approval process will restart.

Requirements for Credit Application and Purchase Orders
To obtain credit approval for all new purchase orders, the following criteria must be fulfilled:
- The customer application below must be completed in full.
- A current W9 form must be submitted by the signer of the customer application.
- The entire order request must be submitted.
- All completed documents should be sent via email, fax, or mail.
